Social Prescribing Link Worker (part time) – maternity cover
Together Co is Brighton and Hove’s loneliness charity, creating connections that change lives. We run befriending and social prescribing services that support over 1000 people a year and have a volunteer team of over 500.
We are looking for a Social Prescribing Link Worker who is confident at managing their own caseload of clients and delivering a quality, creative person-centred social prescribing service, within GP surgeries and/or in patients’ own homes/other venues.

Hours
23-25 hours per week to be agreed with successful applicant (full time = 35 hours a week)
In discussion with relevant manager hours can be worked flexibly within the context of organisational needs being met
Salary
£27,198 per year (Salary Point 18) pro rata + 3% employer’s contribution to auto-enrolment pension
Status
Fixed term – Maternity cover for 6 month period minimum, with possibility of extension
Key skills required
- You will enjoy working collaboratively with others, gaining the trust of the people using our services, colleagues, volunteers and external partners and supporting the achievement of goals.
- You will be both highly organised and flexible, skilled at planning and tracking tasks, appointments and commitments as well as using supporter / customer databases, MS Office applications and other IT systems to keep accurate records.
- You will have an understanding of safeguarding procedures and data protection.
You will have excellent communication skills and enjoy working with a wide range of people.
