Social Prescribing Link Worker
(part time) – maternity cover

Together Co (TC) are Brighton and Hove’s loneliness charity, creating connections to change lives for over 20 years. We do this through award-winning befriending, social prescribing and a range of volunteering programmes.
To support clients and volunteers; to provide a person-centred and holistic social prescribing service, within GP surgeries and other relevant venues. This role will be part of our Primary Care Network (PCN) offer, based primarily in GP surgeries, as well as supporting the wider social prescribing team.

Hours
Up to 28 hours per week to be agreed with successful applicant. Option for less hours per week may be possible, depending on service delivery.
Location
Brighton & Hove – Primarily in GP surgeries within the central & eastern parts of the city.
Salary
£27,198 FTE (SP18) + up to 3% employer’s contribution to pension. (Full time is 35 hrpw.)
Status
Fixed term – Maternity cover for 6 month period minimum with potential for extension.
